How to register as a new patient online

Complete the online form to register to become a patient at Priory Road Medical Centre.

You will need to fill in all of the details to register at our practice. You will also need to provide a copy of your ID and proof of address. For example, valid ID could include a photograph of your passport or driving licence. This is to check that you live in the local catchment area.

We may also ask you for details of your previous GP practice and basic health and medical information.

Register as a new patient online

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Please be aware that you will need to fill in a new patient questionnaire to complete your registration, questionnaires are available from reception.

Alternatively, you can register by completing a form available from reception.

You will need to complete this by hand & ensure you sign the form and bring it to the surgery with 1 form of photographic ID (e.g. Passport, driving license etc) and 1 form of proof of address dated within the last 3 months (e.g. Utility bill, bank statement etc) to support your application.

Although you will be officially registered with one doctor you may see the doctor of your choice. If you wish to see a particular doctor for some or all medical conditions please let the patient co-ordinator know and she will note this in your medical records. Note that while we will endeavour to accommodate this it may not always be possible.

Registering online as a non-English speaker

Registering as a temporary patient

If you are away from home, you can register as a temporary patient with Priory Road Medical Centre for up to three months. This will allow you to use our practice and still remain a patient at your permanent GP practice. After three months, you will have to re-register as a temporary patient or permanently register with us.

Registering children

Children under 16 need to be registered by their parent or guardian. You can do this by completing the online GP practice registration form on their behalf. You will need to provide a form of ID to prove that you’re their parent or guardian.

What to do if you’re registering on behalf of someone else

If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.

Change of address

If you are moving house, please let us know as soon as possible. If you are moving out of the practice’s catchment area, you may be asked to register with a practice that’s closer to your new home address.
